If Assist is required to maintain a formal Notice of Privacy Practices for a service, the operative notice supplied through the relevant care or clinical process controls over this website summary. You may request the current approved notice by calling the home-care team. This page is not a substitute for an approved NPP.
Individual rights
Depending on applicable law and the relationship involved, individuals may have rights concerning access, copies, amendments, restrictions, confidential communications, accountings, and complaints about protected health information. The operative notice explains the relevant rights and process.
Secure communication
Ask the appropriate team for an approved secure method before transmitting any sensitive record. Verify the recipient and purpose. In an emergency, call 911 rather than sending a website message.
